Antibiotic-resistant bacteria are emerging worldwide and contribute substantially to the increasing burden of infectiouse disease in the hospital as well as in the community. A short overview will be given regarding the most important and frequent bacterial species: methicillin – and glycopeptide -resistant staphylococci, glycopeptide -resistant enterococci, penicillin-resistant pneumococci and quinolone-resistant and/or ESBL-producing gramnegative rods. The situation of methicillin-resistant Staphylococcus aureus (MRSA) will be used to demonstate the global dimension and consequences of an “antibiotic resistance epidemic”. Data from recent basic research will be presented which may help to elucidate the various factors contributing to the “success” of the MRSA-epidemic: the appearance of community derived, Panton-valentine-leukocidin-positive MRSA, the biofilm story, the “small colony variant” concept and phenotypic variation, the newly discovered intracellular life and nasal carriage.
