BACKGROUND: Despite numerous limitations, concentrations of serum creatinine and creatinine clearance are widely used measures of renal function and glomerular filtration rate (GFR). Cystatin C (CysC), an inhibitor of cystein proteinases, has been proposed as an alternative marker for GFR. We studied CysC in relation with creatinine and creatinine clearance in renal transplant recipients (RTRs).
MATERIALS AND METHODS: In study group were 71 adult RTRs (M/F=43/28; age range 18–71 years) with 142 serum samples. Concentrations of serum creatinine were determined by Olympus analyser (AU 400) using Jaffe method. Serum CysC concentrations were measured by immunonephelometry on Dade Behring analyser. Creatinine clearance were calculated by the Cockcroft-Gault formula.
RESULTS: In 142 samples, median and range concentrations for CysC and serum creatinine were 1,61 (0,85-10,38) mg/L and 112,5 (61-1115) umol/L, respectively. Serum CysC showed excellent correlation with serum creatinine (r = 0,93; P < 0,001), and 1/creatinine clearance (r = 0,91; P < 0,001). No significant differences were found for linear correlation of creatinine with 1/creatinine clearance (r = 0,95; P < 0,001). Elevated levels of CysC (>0,96mg/L) had 97% (138/142) of analysed samples.
CONCLUSION: Our data show great ability of CysC to reflect creatinine clearance and changes in creatinine concentrations, so CysC allows for rapid and accurate assessment of GFR in RTRs.
