BACKGROUND: The serum level of FT is generally accepted as a parameter for the biologically active testosterone fraction. FT determinations are used mainly for assessment of partial androgen deficiency of the aging male (PADAM) and hyperandrogenic/hypoandrogenic states in females. To date, FT is routinely measured by RIA. However, RIAs are plagued with well known problems such as radioactive waste and instability. Further, FT is known to have complex biological equilibrium kinetics and a high total (biological and technical) imprecision. Thus, a recently published nomogram based on total testosterone (TT), sex hormone binding globulin (SHBG) and albumin and 2 novel ELISAs were tested. METHODS: Serum specimens from 26 males (age: 17 to 79 years; mean: 47, median: 47) and 3 females (age: 25 to 71 years) were tested for FT with the 2 ELISAs (ELISA A: DRG, Marburg an der Lahn; ELISA B: IBL, Hamburg) and compared to a gold standard RIA (DPC, Los Angeles). In addition, TT, SHBG and albumin were determined, and FT concentrations calculated from TT, SHBG, albumin concentrations and appropriate dissociation constants (nomogram). RESULTS: For the 2 ELISAs, the following mean inter-run (intra-run) coefficients of variation were found (range from 1 to 100 pmol/L): ELISA A 19% (11%), ELISA B 8% (3%). Linear regression analysis for the correlation nomogram vs. RIA yielded a r2 value of 0.841, whereas for ELISA A vs. RIA and ELISA B vs. RIA, the r2 values were well below 0.7. The 2 ELISAs also correlated poorly with the nomogram (r2 values well below 0.7). SUMMARY and CONCLUSION: A recently published nomogram using TT, SHBG and albumin for interpolation of FT correlated well with the gold standard RIA for FT. Further, the 2 FT ELISAs evaluated as RIA-alternatives performed significantly less well. Together, the nomogram can be used in PADAM and hyperandrogenic/hypoandrogenic states in females for FT calculation, whereas the use of FT ELISAs needs further clarification.
