Objective: Brain natriuretic peptides (BNPs) are useful in the assessment of heart failure. We investigated the utility of brain natriuretic peptides (NT-proBNP, BNP) in monitoring various types of acute and chronic cardiovascular diseases (chronic heart failure due to coronary artery disease, hypertension, congenital heart disease with ventricular dysfunction). Methods: Roche NT-proBNP assay was performed in 206 patients (mean age 49 years; range 20–89 years) and 48 healthy controls (mean age 43 years; range 13–65 years) and Bayer Shionoria BNP Assay was performed in 50 patients (mean age 52 years; range 20–79 years). Results: NT-proBNP concentrations differed in patients with (n=206) and without (n=48) cardiovascular diseases (mean 1198 pg/ml versus 48.2 pg/ml, p < 0.0001). The best cut off value for cardiovascular diseases was at 129,7 pg/ml (AUC–area under curve =0.9324; sensitivity 97.9 %; specificity 80%). Cut off value for BNP was assigned at 100 pg/ml. Comparison of NT-proBNP and BNP values in patients shows significant correlation (r=0.81; p < 0.0001). Summary: Nineteen patients with structural heart disease (39%) showed positive NT-proBNP levels (above the cut off value 127.9 pg/ml) and negative BNP levels (below 100 pg/ml), one patient (2%) showed positive BNP and negative NT-proBNP levels, twenty eight patients (59%) showed both NT-proBNP and BNP positive levels. Conclusion: NT-proBNP is more sensitive in patients with structural heart disease without heart failure. Brain natriuretic peptides appeared to be useful test for monitoring various types of cardiovascular diseases.
